Minor Release Form
Everyone under the Age of 17 attending the Karting Event needs one of these filled out. All other information applies to all. Parents must sign and if not signed in front WVSO staff, it needs to be notarized. NO EXCEPTIONS. Anyone 17 years old or younger needs one.
